ABSENCES & REQUEST FORM

MYS attendance policy:
MYS musicians may not miss more than 4 rehearsals before any one concert,
or more than 9 rehearsals for the entire season.
Students with more than the allotted number of absences will be excused from upcoming performances.
Appeals may be heard, and a final decision will be made
based on reasons for absence,
dedication to MYS and musicianship.
Absence forms should be submitted 2 weeks prior to a scheduled absence.
If absence is due to an illness or last-minute emergency, please submit the form
as soon as possible.
WE ASK THAT YOU PLEASE DO NOT CALL THE MYS OFFICE TO REPORT ABSENCES.
